Description
ROLE
The Diabetes Dietitian is a member of the interdisciplinary diabetes education team providing evidence based best practices specific to the nutritional care of people with diabetes.
- Assessment of nutritional care needs of the client.
- Develop, implement, evaluate and document nutritional interventions and programs, based on the Stages of Change model and concurrent with the College of Dietitians Standards of Practice.
- Provides nutrition education and follow-up related to diabetes management to clients.
- Collaborate with the interdisciplinary team to provide diabetes education, care and management specifically regarding nutrition.
- Maintains accurate client records as per college, NDHN and organization guidelines regarding the nutrition care process.
- Participates in team conferences.
- Participates in classroom and individual teaching.
- Collects and reports appropriate program statistics.
- Establishes appropriate linkages within the community, sponsoring agency, and appropriate health care agencies to promote primary and secondary disease management strategies, best practices and current treatment modalities.
- Collaborates on initiatives related to diabetes and chronic disease awareness, prevention and management as it relates to healthy lifestyles and nutrition.
- Acts as a nutrition and healthy lifestyles resource to health agencies, community agencies and other internal and external health and social agencies as it relates to diabetes.
REQUIRED EDUCATION & CERTICIATION(S)
- Bachelor’s Degree in Nutrition and Food Science
- Completion of a recognized Dietetic Internship or equivalent
- Current registration with the College of Dietitians of Ontario
- Certified Diabetes Educator designation by the Canadian Diabetes Association preferred or intent to obtain
- Certified Pump Trainer certificate or willingness to obtain
- Membership in Diabetes Educator Section of Canadian Diabetes Association an asset
- Valid driver’s license and access to vehicle preferred
REQUIRED SKILLS & EXPERIENCE
- Previous experience in adult education, patient counselling.
- Minimum of 2-3 years in an interdisciplinary health care setting.
- Minimum of 2-3 previous experience in diabetes education.
- Basic Microsoft Office computer skills.
- Effective communication skills ( oral, written) and the ability to work collaboratively within a team as well as independently.
- Effective critical thinking, problem solving and leadership skills.
- An awareness and sensitivity to the complex nature of diabetes and the social, economic and cultural needs of the population.
- Organizational skills to perform required duties effectively and efficiently
- Regular and punctual attendance is mandatory
DESIRED SKILLS, EDUCATION & CERTIFICATION(S)
- Familiarity with:
- Blood glucose monitoring
- Insulin Pens
- Insulin pumps

Hours of Work: Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements and per the OPSEU Paramedical collective agreement.
