Description
ROLE:
The Patient Flow Navigator (PFN) utilizes their knowledge, skill, and judgement within their nursing scope of practice to assist, when applicable, and facilitates timely, effective discharge planning and efficient patient flow at their affiliated site. In collaboration with the interprofessional health care team and external partners, the PFN promotes effective utilization of resources by reducing and/or eliminating any impediment to effective patient flow. This role also involves admission avoidance in the Emergency Department if a patient is not a Home and Community Care Support Services North Simcoe Muskoka (HCCSSNSM) or Seniors Assessment and Support Outreach Team (SASOT) client.
REQUIRED EDUCATION & CERTIFICATIONS
- Bachelor of science in Nursing.
- Current Registration in good standing with the College of Nurses of Ontario.
- Current BCLS (Basic Cardiac Life Support).
REQUIRED SKILLS & EXPERIENCE
- Minimum three years of hospital nursing experience in an acute care setting.
- Demonstrated ability to communicate effectively and professionally with patients, families, caregivers, and other professionals.
- Demonstrated ability to work effectively with others in a positive, collaborative, collegial manner, demonstrating innovative leadership and complex decision-making skills with the ability to negotiate and effectively interact as a team member in an interdisciplinary setting.
- Demonstrates sound judgment, flexibility and initiative using tact, diplomacy, and discretion.
- Working knowledge of hospital policies/procedures and protocols.
- Demonstrated ability to lead projects and initiatives.
- Actively participates in committee work.
- Promotes and provides patient / family-centred care.
- Excellent computer skills having worked with Microsoft Office Suite and electronic health records programs (i.e., CERNER)
- Highly motivated and flexible with the ability to adapt quickly to new situations/challenges.
- Demonstrated appreciation and respect for protocol, professionalism, and confidentiality.
- Demonstrated organizational and time management skills.
- Awareness of community supports and services for identified populations would be an asset.
DESIRABLE SKILL & EDUCATION
- Patient flow experience including discharge planning experience.

Hours of Work: Must be available to work shifts including days, evenings, nights and weekends. Shifts and hours may change according to departmental requirements and as per the ONA collective agreement.
